Job Summary
Kumukahi Health + Wellness is seeking a dedicated and compassionate Medical Case Manager to join our team and help improve the lives of individuals living with HIV in our East Hawaii Office. This role is essential in providing direct support to patients in managing their healthcare, accessing services, and ensuring that patients are connected to the resources they need for comprehensive care. The HIV Medical Case Manager will collaborate with a multidisciplinary team to create personalized care plans and ensure timely interventions for optimal patient outcomes.
Duties & Responsibilities​

Provide comprehensive case management services to individuals living with HIV, including coordinating medical, social, and mental health care.

Conduct thorough assessments to evaluate the needs of patients and develop individualized care plans.

Ensure access to HIV-related medical treatments, medications, and necessary diagnostic tests.

Facilitate the coordination of care with healthcare providers, social workers, mental health professionals, and other community services.

Assist patients in navigating the healthcare system, including insurance and financial assistance programs.

Educate patients about HIV treatment options, prevention strategies, and maintaining adherence to antiretroviral therapy (ART).

Monitor patients’ progress and adjust care plans based on health outcomes and changing needs.

Provide ongoing emotional support and counseling resources to patients and their families.

Advocate for patients to ensure they receive appropriate care and services.

Maintain up-to-date patient records and documentation in compliance with confidentiality standards and regulatory requirements.

Participate in case reviews and team meetings to discuss patient progress and care coordination.

Qualifications

Bachelor’s degree in Nursing, Social Work, Public Health, or a related field preferred

1-3 years of experience in case management or healthcare, with a focus on HIV care, preferred. This can include experience in hospitals, outpatient clinics, community health centers, or other healthcare settings where HIV services are provided.

Experience working with individuals living with HIV or other chronic conditions is highly preferred.

Certification as a Case Manager (CCM) or related credential is a plus.

Knowledge of HIV-related medical treatments, community resources, and support systems.

Strong communication skills with the ability to work effectively with diverse populations.

Compassionate and nonjudgmental approach to working with patients.

Excellent organizational and problem-solving skills.

Ability to work both independently and as part of a team.

Knowledge of HIPAA regulations and maintaining patient confidentiality.

Must be comfortable with electronic health record (EHR) systems and case management software.
