Full-Time HIV/STI PrEP Navigator

The HIV/STI Pre-Exposure Prophylaxis (PrEP) Navigator provides HIV, STI, and Hepatitis C counseling and testing, intensive care coordination, support, and services to Hawai’i Island individuals who require assistance in accessing and remaining in HIV and STI PrEP care. This individual will coordinate with the Prevention team to initiate access to PrEP care and the Medical and Pharmacy teams to assist existing PrEP patients.

Full-Time HIV Clinic Medical Case Manager

In this HIV Clinic Medical Case Manager role, you will be an essential element of an integrated care team that recruits, assesses and qualifies patients for case management services at Kumukahi Health + Wellness. You will be under the Director of Care Services, but also have some clinical responsibilities in assisting the Medical Provider with the sexual health clinic in our both Hilo & Kona office.

Medical Case Manager

In this Medical Case Manger role, you will be an essential element of an itegrated care team that recruits, assesses and qualifies patients for case managerment services at Kumukahi Health + Wellness (KHW). You will be under the Director of Care Services and be responsible for completing biannual reassessments with clients to include completion of a care plan in addition to seeing clients periodically to address their current needs and checking compliance to medical care and adherence HIV medications. This position maintains a cohesive and productive relationship established from a client-centered perspective.

Patient Service Representative

Kumukahi Health + Wellness seeking to hire a full-time Patient Service Representative (PSR) to join our dynamic team of health advocates. The PSR is part of the administrative call center team and answers all incoming agency phone calls from patients, insurance providers, referral agencies, and any other calls directed to the agency. The PSR reports to the Clinic Manager and is responsible for providing excellent and accurate customer service in a friendly manner. The PSR helps to maintain efficient workflow in administering day-to-day patient scheduling, appointment reminder confirmations, plus other duties as assigned by the Clinic Manager including checking voicemail messages, returning customer calls, verifying patient insurance and home address, and confirming PCP designation.
