In this HIV Clinic Medical Case Manager role, you will be an essential element of an integrated care team that recruits, assesses and qualifies patients for case management services at Kumukahi Health + Wellness. You will be under the Director of Care Services, but also have some clinical responsibilities in assisting the Medical Provider with the sexual health clinic in our both Hilo & Kona office.

Your primary roles include: carrying a caseload of HIV positive clients possibly with medical co-morbidities, qualifying patients for Ryan White services, conducting a thorough nursing assessment of clinic patients and carrying out provider's order, creating and updating acuity and treatment plans, and monitoring HIV labs, including viral load. Case management services may entail securing medical transportation to/from appointments, assisting patients towards treatment readiness, facilitating telehealth visits, and other activities that ensure treatment successes.

  • Understanding of Hawai'i Island geography and its unique populations in both East & West Hawaii

  • Must be able to communicate and function productively on an interdisciplinary team of various educational and professional backgrounds

  • LPN, MA OR CNA preferred

  • Medical charting experience desired with high attention to detail

  • Must provide services to patients in a manner that is non-judgmental, client-centered and strengths-based

  • Must be comfortable with Harm Reduction in underserved populations, such as sex workers, people without housing, and patients who actively use illegal substances

  • Experience working with LGBTQ and/or HIV individuals preferred

  • Experience in referrals to community resources

  • Familiarity with medical insurance in Hawai'i

  • Ability to maintain the highest standards of patient confidentiality due to the sensitive nature of our patient base

  • Valid Hawai'i Driver’s License, clean Drivers Abstract required plus reliable transportation

  • Provide proof of Covid-19 vaccination upon hire

Must be able to maintain complete confidentiality of medical records, patient information, and office memoranda. Personal characteristics to include: a team player, high integrity, communication and customer relations skills, regular work attendance, courteous and friendly, able to work well with diverse groups of people, and gain and maintain respect of others, both inside and outside KH+W.

Because the employee has access to personal and professional data regarding individual employees and their families, personal data regarding individual patients and the families, and/or sensitive company financial data, an essential job result is the maintenance of a high level of confidentiality of the information processed by the employee. Violation of this confidence may result in disciplinary action, up to and including termination of employment.

This position description is not all-inclusive of the duties that may be assigned to the employee. In order to ensure maximum flexibility and efficiency and to encourage cross training, employees will be assigned additional duties as deemed necessary or desirable by KHW.

Working Conditions

Indoors office, air-conditioned rooms. Travel to meetings and other program facilities as necessary.

Work Hours

Monday – Friday, 8:30 am – 4:30 pm (Unpaid Lunch hour 12 pm – 1 pm), 35 hour work week
