The HIV/STI Pre-Exposure Prophylaxis (PrEP) Navigator provides HIV, STI and Hepatitis C counseling and testing, intensive care coordination, support and services to Hawai’i Island individuals who require assistance in accessing and remaining in HIV and STI PrEP care. This individual will coordinate with the Prevention team to initiate access to PrEP care and the Medical and Pharmacy teams to assist existing PrEP patients.

This individual works within the STI/HIV testing team and applies a client-centered approach with individuals on Hawai'i Island, with a targeted focus on those utilizing Kumukahi Health + Wellness Aiaola Sexual Health Clinic. This position will assist PrEP clients with services such as, but not limited to, health insurance enrollment, access to supportive co-pay programs, PrEP provider identification, appointment scheduling, appointment reminders, completion of lab work, medication access, ensure transportation to and from PrEP appointments, education on the benefits of PrEP and ongoing maintenance.

This position will work to empower clients, with a targeted focus on men who have sex with men, transgender women, Native Hawaiians and other marginalized communities at risk of HIV or STI exposure to navigate medical and social systems on their own.

  • Work with Prevention staff and others to identify potential PrEP clients and those needing PrEP navigation services.

  • Provide support to HIV-negative individuals who are interested in starting PrEP and those already utilizing PrEP care.

  • Provide support to individuals who are interested in starting STI PrEP interventions, such as Doxy-PrEP.

  • Complete screenings to identify readiness for PrEP and other barriers to care. Work with clients to identify position solutions.

  • Assess benefits and insurance, and if needed, provide assistance to access payor programs.

  • Provide PrEP disease prevention and PrEP adherence education and counseling.

  • Effectively collaborate with all individuals involved in PrEP care and program delivery to support high-quality PrEP programming.

  • Make referrals for external services needed to support PrEP care and follow up to ensure service delivery.

  • Provide ongoing follow-up to ensure PrEP adherence.

  • Consult with the Aiaola Sexual Health Clinic to provide transportation to and from PrEP appointments.

  • Maintain appropriate patient files and timely documentation of services utilizing electronic prevention or medical software.

  • Attend weekly ASH huddles to report on PrEP patient appointments and progress.

  • Serve as a fully functioning member of the Prevention team, able to participate in daily activities and functions.

  • Provide outreach to relevant communities with a focus on men who have sex with men and transgender women community.

  • Establish working relationships with appropriate community resources in the area, including those providing mental health and social services to the target population.

  • Any and all other duties assigned by your supervisor(s).

  • Possess a high school diploma or GED equivalent.

  • Ability to exercise tact, diplomacy, and discretion, and to maintain and sustain confidentiality in relationship to clients.

  • Experience working with and cultural sensitivity and knowledge of diverse populations, including Native Hawaiian, Hispanic, LGBTQIA+, and communities of different cultures.

  • General knowledge of health promotion and HIV, STI, and viral hepatitis prevention strategies.

  • General knowledge and understanding of evidence-based interventions (behavioral risk assessment).

  • Communicate effectively using both verbal and written skills.

  • Valid driver's license and personal transportation are required.

  • Self-directed, motivated, and flexible with the ability to work independently and as part of a team.

  • Ability to manage multiple tasks and responsibilities.

Working Conditions

Indoors office, air-conditioned rooms. Travel to outreach events, meetings and other program facilities as necessary.

Work Hours

Monday – Friday, 8:30 a.m. – 4:30 p.m., Some nights and weekends

Equipment Use

Standard office equipment; use of own vehicle required.

Physical, Mental, and Communication Demands

Requires use of discretion and judgment; requires working to established procedures, maintaining reliable work attendance; may require working under minimal supervision; requires verbal communication skills and following oral and written instructions.

Skills/Knowledge

In addition to meeting the mental, physical, and communication demands listed above, must be able to read, speak, and comprehend English, use computers, climb stairs, and lift 30 lbs.

A valid driver’s license required and clean driving record required.
